About The Position

This position is with the Defense Health Agency, CBMH Puyallup, DIR of Branch Clinics Army Health Clinic Family Medicine Service, in Puyallup, Washington. This is a Direct Hire Solicitation Madigan’s Patient-Centered Medical Home Primary Care Clinics are Army Medical Homes with an interdisciplinary approach to delivering evidence-based, comprehensive primary care– coordinating care delivered outside of the primary care setting and proactively engaging patients as partners in health. The Army Medical Home is designed around one core principle: putting patients first. The Army Medical Home is Army Medicine’s gateway to influence the “life space” where patients make decisions on the key determinants of health and wellness–sleep, activity, and nutrition (that is, the Performance Triad). Each patient will partner with a team of health care providers - physicians, nurses, behavioral health, clinical pharmacists, physical therapy, and case management professionals - to develop a comprehensive, personal health care plan. This team will form a partnership with you to provide improved access, coordinate needed services, answer your questions, and ensure you get the care you need. Your Medical Home team will remain the same as long as they continue to work within your medical home. This will improve your continuity of care and allow you to develop a closer partnership with your team. The Army Medical Home model has been shown to improve patients’ overall health, resulting in fewer emergency and urgent care visits decreased hospital admissions, shorter inpatient stays, and fewer inpatient readmissions among other positive results. The Puyallup Community Medical Home refers to facilities serving the Puyallup area, primarily the Madigan-Puyallup Community Medical Home (part of Madigan Army Medical Center) located off 156th St E on South Hill, offering primary and specialty care, and also the Puyallup Medical Clinic (Community Health Care) at 201 W Main St, a downtown location focused on community health needs. Both provide vital local healthcare, but Madigan serves military families (TRICARE) and the other serves the broader public. Requirements

  • Degree: Doctor of Medicine, Doctor of Osteopathic Medicine, or equivalent from a school in the United States or Canada. This degree must have been accredited by the Council on Medical Education of the American Medical Association; Association of American Medical Colleges; Liaison Committee on Medical Education; Commission on Osteopathic College Accreditation of the American Osteopathic Association, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. A Doctor of Medicine or equivalent degree from a foreign medical school must have provided education and medical knowledge equivalent to accredited schools in the United States. Evidence of equivalency to accredited schools in the United States is demonstrated by permanent certification by the Educational Commission for Foreign Medical Graduates, a fifth pathway certificate for Americans who completed premedical education in the United States and graduate education in a foreign country, or successful completion of the U.S. Medical Licensing Examination.
  • Licensure: Applicants must possess a current, active, full, and unrestricted license or registration as a Physician from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
  • Graduate Training: Subsequent to obtaining a Doctor of Medicine or Doctor of Osteopathic Medicine degree, a candidate must have had at least one year of supervised experience providing direct service in a clinical setting, i.e., a one-year internship or the first year of a residency program in a hospital or an institution accredited for such training.
  • Residency Training : Applicants must have successfully completed the required accredited residency training in the specialty of the position to be filled (Family Practice, Family Medicine) and equivalent experience and training equal to at least 4 years
  • FOREIGN EDUCATION: If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university.

Responsibilities

  • Provide comprehensive primary care using independent clinical judgment, including diagnosis, treatment, prevention, and referral across medical, surgical, obstetric, gynecologic, and orthopedic conditions.
  • Manage an assigned patient panel and ensure coordinated, continuous, patient-centered care across multidisciplinary teams and all elements of the healthcare system.
  • Document all clinical services accurately and in accordance with regulations, policies, and established procedures.
  • Teach and consult with other healthcare providers and participate in Continuing Medical Education, quality assurance, and performance improvement activities.
  • Supervise assigned healthcare personnel, including assigning work, providing technical oversight, evaluating performance, interviewing candidates, recommending personnel actions, and addressing training and disciplinary needs.
  • PERFORMS OTHER DUTIES AS ASSIGNED.
