About The Position

VHA Home HealthCare (VHA) is seeking a Supervisor for the Toronto Safer Spaces: Hoarding Support program. This role leads a specialized staff team that supports vulnerable clients facing hoarding challenges that jeopardize their housing. The position involves managing referrals, conducting initial client visits and assessments, developing care plans, and supervising a team of Mental Health Case Workers and Modified Cleaners. The Supervisor will also respond to inquiries about VHA services, provide information and referrals, and participate in quality initiatives, ensuring all activities align with VHA's policies, procedures, and confidentiality standards.

Requirements

  • BSW or MSW Degree in Social Work, Addictions or equivalent combination of education and experience.
  • Minimum of 5 years of experience working with clients with complex mental health issues in a community setting
  • Minimum 3 years of Supervisory experience
  • Experience providing case coordination to vulnerable adults with hoarding, mental health and addictions challenges in home and community setting.
  • Valid driver’s license and use of a car on the job is a requirement.

Responsibilities

  • Receiving and processing referrals from Coordinated Access (City of Toronto)
  • Conducting initial visits and assessments with all clients
  • Reviewing care plans set by Toronto Hoarding Supervisor/Referral and Coordinated Access/City of Toronto
  • Assigning, scheduling, supporting, and monitoring a team of Mental Health Case Workers specialized in working with clients with Lived experience with Hoarding as well as Modified Cleaners
  • Responding to inquiries regarding VHA Services and providing information and referrals
  • Participating in VHA services and quality initiatives
  • Ensuring all activities are conducted within the policies, procedures, and VHA’s standards of confidentiality
  • Responds to all inquiries and provides information on VHA’s services, explains VHA services, and policies.
  • Develops a service plan with the client and in collaboration with the client’s care team.
  • Ensure that funder targets are met.
  • Maintains ongoing communication with client care team throughout service duration and maintains transparency in client’s progress.
  • Conducting Field Supervision on all clients receiving services
  • Schedules initial home visit to review and discuss care plans.
  • Develops and discusses discharge planning with clients and/or referral sources.
  • Conducts on-site supervision and assessment on all Toronto Safer Spaces: Hoarding Support clients funded through City of Toronto.
  • Provides leadership, supervision, training and support for Hoarding Case Workers.
  • Ensures quality and performance standards are monitored and adhered to.
  • Conducts site visits with staff regularly to ensure service delivery plans are carried out to ensure contract compliance
  • Works together with the Manager to develop ideas and recommendations for new programs, enhanced services and productivity improvements.
  • Participates in identified team meetings with a commitment to contribution and active participation in team and organizational quality improvement processes.
  • Participates on project teams as appropriate.
  • Understands the financial aspects of care and is accountable for effective, efficient, and responsible use of time and resources when delivering care.
  • Works to achieve targets set in relation to Agency and funder objectives.
  • Reports to management summarizing intake and case load activity.
  • Participates in service evaluation and development.
  • Documents trends and service requests to provide input for future planning.
