About The Position

The University of Rochester is seeking a Supervisor, Social Worker for their Pediatric Inpatient Psychiatry unit. This role is responsible for coordinating continuity of care and discharge planning, providing general oversight and supervision to social work staff, and ensuring the delivery of services on the unit. The position also involves developing and maintaining administrative liaisons with community agencies and referral sources to facilitate patient care and timely discharge. The Social Worker Supervisor will provide social work services to pediatric patients and their families, participate in interdisciplinary treatment teams, conduct psychosocial and family assessments, and coordinate the discharge planning process. They will also provide individual, family, or group treatment and mobilize community resources. Requirements

  • Master's degree from an accredited school of Social Work and 2 years of professional social work practice required Or equivalent combination of education and experience
  • LMSW - Licensed Medical Social Worker - State Licensure as defined by the State Board for Social Work, State Education department, Division of Professional Licensing Services upon hire required

Responsibilities

  • Supervises the delivery of social work services on the Pediatric Inpatient Psychiatry Unit.
  • Implements patient care, discharge planning, and professional practice policies and procedures in collaboration with the SW Manager, Social Work Division, Department, and Hospital.
  • Participates in recruitment, training, and evaluation of Inpatient social work staff in collaboration with appropriate interdisciplinary members of the program.
  • Conducts weekly meetings of the inpatient social work staff, which may consist of either informal/formal individual supervision or within treatment team settings.
  • Collaborates regularly with the Pediatric Inpatient Leadership Team in the identification and resolution of problems within the delivery of care system with special emphasis on discharge planning for patients on Pediatric Inpatient Psychiatry.
  • Identifies and coordinates safe and timely and comprehensive discharge plan with treatment team, patients/families to include any safety planning and continuity of care for CPEP Pediatric Boarders that need Social Work services while awaiting placement on Inpatient.
  • Identifies contacts and coordinates with outpatient providers/agencies to facilitate continuity of care and discharge planning for the patient.
  • Provides education to patients and families related to internal and community resources and referrals, advocates and supports patients and their families in their efforts to interact with complex social service agencies involved in their care.
  • Documents interventions in compliance with Division, Department and Hospital Policy.
  • May provide consultation support on pediatric disposition cases in the system in collaboration with the SW Manager for Inpatient Psychiatry.
  • Develops administrative liaisons with community agencies including but not limited to OMH, CPS, SPOA, OPWDD and referral resources to facilitate patient movement from the inpatient service to other locations, including home and/or community, in collaboration with SW Manager for Inpatient Psychiatry.
  • Participate in regularly scheduled meetings with community agencies for community planning and support.
  • Attends Social Work Division and Department Staff meetings, as required.
  • Participates in activities of the Social Work Division including but not limited to timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews.
  • Subscribes to the Departmental goal of creating a culturally competent environment by treating patients, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.
  • Participates in available cultural competence events and training appropriate to job duties.
  • May provide social work coverage and back-up as needed during the absences of Pediatric Inpatient Psychiatry social work staff.
  • Other duties, as requested and assigned.
