Individual salaries will be determined based on factors such as market data, education, experience, qualifications, expertise, and internal equity.

Requirements

  • Master's degree from an accredited school of Social Work and 2 years of professional social work practice required Or equivalent combination of education and experience
  • LMSW - Licensed Medical Social Worker - State Licensure as defined by the State Board for Social Work, State Education department, Division of Professional Licensing Services upon hire required

Responsibilities

  • Collaborates with the Social Work Clinical Coordinator and Clinic Director in the ongoing management and operations of Adult Ambulatory Crisis and Access coordination.
  • Serves as a member of the Leadership Team, participating in the development, implementation, and ongoing evaluation of initiatives to effect integrated, collaborative, patient-centered, recovery orientated care.
  • Coordinates timely access to care for individuals referred for services.
  • Provides clinical and administrative supervision of assigned social workers, LMHC’s, LMFT’s, or other disciplines as appropriate.
  • Provides clinical services for patients including individual, family, and group therapies, crisis intervention, and community outreach as part of an interdisciplinary team.
  • Provides direct clinical services to assigned patients, which may involve individual, family, and/or group therapy, crisis intervention, and diagnostic evaluations.
  • Engages in consultation, treatment planning, and periodic care review.
  • Collaborates with family, social supports, and community agencies to ensure appropriate and effective care.
  • Coordinates referrals and facilitates linkage to other appropriate agencies/supports.
  • Provides administrative and clinical supervision to therapists consistent with Clinic and Department standards.
  • Participates in policy and procedure development for the Clinic, establishment of initiatives to improve positive patient care outcomes, and provides teaching to Clinic staff.
  • Provides telephone screening and assessment to individuals wanting to access services at Adult Ambulatory Clinic.
  • Maintains real-time handle on clinic schedules and flow of patients through the system.
  • Facilitates interdisciplinary treatment team meetings as well as other general staff/therapist meetings.
  • Assists in recruitment and evaluation of performance of therapists including interviewing, quarterly audits of the electronic medical record, and yearly performance evaluations.
  • Documents interventions, both direct and indirect, in the patient’s medical record in compliance with Division, Department, and Hospital policy.
  • Attends Division, Department, and Clinic level staff meetings.
  • Ensures that Division, Department, and Clinic policies and procedures are implemented with regard to professional practice and patient care.
  • Ensures timely completion of statistical reports/forms, time sheets, and other Hospital or Division mandatory in-service trainings and Health Assessments.
  • Maintains a culturally competent environment by treating patients, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.
  • Participates in available cultural competency events and training appropriate to job duties.
  • Performs other duties as assigned.
