About The Position

The Resource Supervisor serves as a pivotal leader for the Care Coordination Department, providing day-to-day supervision of resource coordination activities and staff while managing, tracking, and coordinating daily throughput efforts to optimize patient flow across the healthcare continuum. This position oversees workflow prioritization, assignment distribution, and timely follow-up on discharge barriers, and functions as an escalation point for complex patient flow and discharge concerns. The Supervisor provides coaching, mentoring, and performance feedback to team members; supports onboarding and competency development; and promotes adherence to departmental policies, regulatory standards, and payer requirements. This role collaborates with interdisciplinary teams—including nursing, physicians, case management, and leadership—to address discharge delays, identify opportunities for process improvement, and ensure efficient and timely discharge of patients.

Requirements

  • BSN required; MSN preferred.
  • New Jersey Registered Nurse licensure required.
  • Minimum of three years clinical nurse experience.
  • Minimum of 3 years of discharge planning experience required.
  • Minimum of 2 years of recent case management experience required.
  • Ongoing experience with physician and clinical staff practice patterns and operational issues.
  • Registered Nurse License Required.
  • Must have knowledge and understanding of Centers for Medicare & Medicaid Services (CMS) guidelines and Conditions of Participation.
  • Ability to work independently, setting priorities to coordinate care plans efficiently.
  • Ability to coordinate, compile, and analyze data.
  • Demonstrated ability to lead and develop staff through coaching, feedback, and conflict resolution, supporting a culture of accountability and service.
  • Excellent communication skills (oral and written) and interpersonal skills necessary to negotiate with families, patients, post-acute providers and third-party payors.
  • Understanding of chronic disease management, rehabilitation, insurance processes, community resources, and hospital policies.
  • Familiarity with regulations such as HIPAA and the Affordable Care Act.
  • Strong communication, critical thinking, problem-solving, collaboration, empathy, organizational, and time management skills.
  • Adaptability to rapidly changing situations, new information, and complex cases is necessary for optimal team support.
  • Managing challenging scenarios and emotional demands with professionalism and composure.

Nice To Haves

  • MSN preferred.
  • Prior supervisory/charge/lead experience (formal or informal) preferred.
  • National certification in Case Management preferred; required within 2 years of hire.
