Requirements

  • Registered Nurse Diploma
  • Current registration in good standing with the College of Nurses of Ontario as a Registered Nurse and member of RNAO.
  • Current Basic Life Support (BLS) certification with Healthcare Provider CPR Level C.
  • Minimum five years’ experience in long-term care with advanced clinical knowledge in nursing practice.
  • In-depth knowledge of the Fixing Long Term Care Act 2021 required programs.
  • Current CPR certification.
  • Strong leadership, interpersonal and communication skills and the ability to work both in a team and independently.
  • Proven ability to coach, mentor and support staff in continued development of clinical skills, critical decision making and resident centered care.
  • Experience in training and group facilitation and/or adult education.
  • Strong analytical skills, decision-making skills and the ability to manage multiple demands.
  • Proficient computer skills with word, excel and electronic document software.

Nice To Haves

  • Supervisory/leadership experience.
  • A Bachelor of Science in Nursing degree.

Responsibilities

  • Provide education and coaching to registered team members on required clinical programs, management of clinically complex/high risk residents and related policies and procedures.
  • Supervise, coach, mentor and overall performance management of Resident Services Coordinator and RAI Coordinator.
  • Assist with the coordination and orientation of new hires/students/volunteers.
  • Develop and deliver education for ongoing clinical education as evident from compliance reports, audits and clinical needs.
  • Develop and evaluate care delivery processes to continually improve nursing practice through direct observation and process audits.
  • Coordinate clinical policy and program change roll out at home level to support knowledge transfer.
  • Support frontline staff in caring for complex care residents by facilitating the development of a comprehensive care plan, participating in care planning rounds, post critical incident debriefing and/care conference for high risk admissions as assigned by senior nurse manager.
  • Responsible for CQI audits/ program evaluations for required clinical practices in collaboration with Manager of Resident Care/ Committee leads.
  • Perform other duties as assigned.
