About The Position

Are you a collaborative healthcare professional with a passion for patient care, leadership, and care coordination? Join our team as a Supervisor, Rehabilitation Case Management, where you'll play a key role in supporting both patients and care teams while helping drive the success of our Case Management department. In this position, you will assist with the day-to-day operations of the Rehabilitation Case Management team and partner closely with the department manager to achieve departmental goals and ensure high-quality patient care. You will maintain your clinical skills by managing a small caseload while serving as a trusted clinical resource and mentor to case management staff. Working collaboratively with physicians, nurses, and interdisciplinary team members, you will help ensure patients receive timely, effective, and coordinated services. You will also assist in identifying and resolving barriers to care, supporting positive patient outcomes and an exceptional care experience.

Requirements

  • Current clinical licensure as required for the role.
  • Experience in rehabilitation, case management, or a related healthcare setting.
  • Strong leadership, communication, and problem-solving skills.
  • Ability to work effectively in a collaborative, patient-centered environment.
  • Licensed Clinical Social Worker Required
  • Master's Degree Social Work Required
  • 3 Years Clinical - Case Management Required

Nice To Haves

  • Certified Case Manager (CCM) credential required within one year of employment.
  • 1 Year Leadership Preferred

Responsibilities

  • Support daily operations of the Rehabilitation Case Management Department.
  • Partner with department leadership to achieve departmental objectives and performance goals.
  • Maintain clinical competency through management of a partial patient caseload.
  • Provide guidance, education, and clinical support to case management staff.
  • Collaborate with physicians, nurses, and interdisciplinary teams to promote timely, quality patient care.
  • Assist in resolving care coordination challenges and service barriers.
  • Serve as a clinical and operational resource for the department.
