This job assumes responsibility for coordinating Clinical Pharmacy Services at Long Term Care Facility. This job facilitates the development, implementation, and maintenance of systems that promote desirable patient outcomes through the use of safe, cost-effective drug therapies. This job provides clinical consultation and clarification to practitioners, provides training, education and competency assessment of pharmacists, clinical rotation students/residents (if applicable) regarding the department's clinical programs, and reviews the performance and documentation of clinical activities of all pharmacists. This position will assume role of Pharmacist in Charge. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
