About The Position

Imagine Pediatrics is a tech-enabled, pediatrician-led medical group dedicated to improving care for children with special healthcare needs. They offer 24/7 virtual-first and in-home medical, behavioral, and social care, collaborating with families, providers, and health plans to overcome access barriers. The organization aims to enhance existing care teams by providing an additional layer of support with compassion and innovation. The primary location for this role is remote, with potential for quarterly travel. The expected schedule is 8:00 am - 5:00 pm Mountain Time (preferred) or Central Time.

Requirements

  • High School Diploma or equivalent
  • Current accredited pharmacy technician certification (such as PTCB)
  • Maintenance of certification expected
  • Registration with applicable state board(s) of pharmacy as required by law
  • Maintenance of all technician registration for all assigned states expected
  • 5+ years of experience as a certified pharmacy technician
  • 3+ years of experience in people leadership
  • Strong Microsoft suite proficiency
  • Proficiency with electronic health record systems

Nice To Haves

  • Experience in a call center or an ambulatory clinic setting, highly preferred
  • Leading pharmacy technicians in a call center environment highly preferred
  • Experience and/or team management in a virtual environment, preferred
  • Bilingual in Spanish and English highly preferred

Responsibilities

  • Oversees and coordinates medication-related services to improve outcomes for pediatric patients with chronic medical and behavioral health conditions.
  • Provides people leadership to the Medication Support Navigator team, including shift scheduling, performance management, and staff development.
  • Works closely with the Clinical Pharmacy Manager to ensure operational excellence and staff competency, facilitate process improvements, and quality oversight.
  • Maintains hands-on involvement in patient care by collaborating with Clinical Pharmacists, Medication Support Navigators, and cross-functional care teams for care coordination.
  • Maintains patient medication therapy records.
  • Troubleshoots medication access barriers with community pharmacies and pharmacy benefits managers.
  • Conducts outreach to patient caregivers.
  • Functions in the Medication Support Navigator - Care Coordinator role supporting both regions and the Medication Navigator team.
  • Coordinates referral tasking to Medication Support Navigator team members.
  • Ensures timely completion of referrals and supports the Medication Support Navigator team in timely completion.
  • Interacts with cross-functional teams for patient care.
  • Facilitates appointment scheduling for pharmacists.
  • Follows referrals and follows up on assignments for rebalancing and in support of daily operations.
  • Leads AM huddle with Clinical Pharmacy Manager.
  • Facilitates operational improvement in Medication Support Navigator processes, in coordination with the Clinical Pharmacy Manager.
  • Serves as Subject Matter Expertise for Medication Support Navigator operations and processes.
  • Maintains Medication Support Navigator shift schedule, time & attendance, PTO management.
  • Directs people leadership of the Medication Support Navigator team including 1:1s and performance management.
  • Ensures operational excellence for the Medication Support Navigator team through monitoring individual contributor performance metrics and quality checks.
  • Onboards new Medication Support Navigators.
  • Supports Clinical Pharmacy Manager to ensure initial and ongoing competency of Medication Support Navigator team members.
  • Performs other duties as assigned.
