About The Position

Manage a team of perinatal community health workers by objectively measuring contact performance, diagnosing problems and creating improvement plans. Develop/maintain a coaching model used to track employees performance (quality, production, attendance, etc.) and development. Understand program performance statistics and reporting systems available and work closely with the Manager/Director to apply them in managing service levels and staff performance. Be available to the staff to answer questions, take escalated calls, and monitor performance measures, etc. Motivate and encourage representatives through positive communication and feedback.

Requirements

  • Bachelors degree in a related field, 6 years of additional experience can be substituted in lieu of degree.
  • Plus 3 years experience as a Community Health Worker, Case Manager, Peer Support Specialist, Care Coordinator, or related position.
  • Travel required.

Nice To Haves

  • 2 years supervisory experience preferred.

Responsibilities

  • Recruit, select, and hire staff using established organizational tools to build effective teams.
  • Administer performance management for the work unit to achieve positive department and organizational performance results with a focus on customer service using organization and time management skills to effectively coach and develop the work unit.
  • Develop, recommend, and implement appropriate social determinant, substance use, and health promotion programs to increase awareness and knowledge relating to the improvement of health on an individual and family basis
  • Drive quality and production outcomes to meet departmental/organizational standards and expectations.
  • Implement and enforce established policies and procedures.
  • Work with delivery sites to incorporate continuous quality improvement into workflows
  • Monitor and communicate to team the gaps in resources available to patients
  • Provide feedback and respond to direction on process improvement due to the innovative nature of peer teams
  • Update community and behavioral health programs, consistent with changing healthcare trends and technology
  • Continuously demonstrate excellence in customer service to include accessibility, research, providing options, and timeliness and follow through.
  • Role model and mentor others to deliver customer service that meets/exceeds customer expectations, results in high member satisfaction scores, and consistently "earns the letter" from members and their families.
  • Demonstrates community health and peer support knowledge to the team with a focus on continuous learning.
  • Interprets data/reports and develops team and individual improvement strategies.
  • Works effectively with cross-functional departments and external customers.
  • Engages and supports community health workers and peer support specialists by promoting and participating in Department initiatives and being accountable for achieving department business objectives.
  • Traveling to community health worker and peer support specialist sites expected.
  • Performs other duties as assigned.
