About The Position

This Supervisor will oversee a team of Care Coordinators and Engagement Coordinators who provide Care Coordination outreach and services to our unengaged member population. Medica’s Supervisor is responsible for managing teams of care coordinators and engagement coordinators for the provision of care coordination services for State Public Program and other Medica members. The majority of members will be from our Care Coordination Products (MSHO, MSC+, SNBC) within State Public Programs and the Government division. In addition, this leader will partner with our Clinical Care Manager in the delivery and evaluation of Medica Care System care coordination services, implementing improvement opportunities, managing essential care coordination duties and oversight of regulatory and quality compliance requirements.

Requirements

  • RN Diploma or Degree in Nursing (Associates or Bachelors) + a minimum of 2 years’ experience in community based services, OR Bachelor's degree in Social Work + a minimum of 1-2+ years of community based services
  • 5+ years clinical experience
  • 1-2+ years supervisory experience of professional staff, including employees who are home office or located remotely; informal leadership that has included coaching and mentorship will also be considered
  • Active, unrestricted RN OR SW License in the state of MN Required
  • Excellent customer service skills and proven team building skills
  • Excellent computer skills and ability to leverage technology to lead remote teams
  • High degree of initiative and ability to work independently, and within a group
  • Ability to plan, organize and prioritize work effectively, including the flexibility to accommodate frequent changes
  • Strong problem solving skills
  • Consultant skills to articulate business needs and translate to quality programs that result in targeted staff competencies
  • Strong leadership skills

  • MnCHOICES Certified Assessor Training (MNCAT) preferred or ability to obtain within 21 days of employment
  • Public Health Certification preferred, however not required
