About The Position

Mainstay Supportive Housing and Home Care, Inc. provides human service and housing programs across Massachusetts, including permanent supportive housing and compassionate, person-centered home care. Our programs have helped hundreds of people live productive and stable lives in their own communities every day for all of these years. Mainstay was founded in 1975 to provide services to people with mental illness, and our mandate has since expanded to cover a variety of vulnerable populations, including low-income elders; people with intellectual, developmental, and physical disabilities; and the recently homeless. Join us in bringing meaningful, programmatic support to individuals who have been historically underserved and marginalized, including those with experiences of alcoholism, addiction, and homelessness. Our BRIDGE Program—Building Resilience, Independence, Dignity, and Growth Every Day—is grounded in compassion, respect, and a commitment to empowering people to live more stable lives in their communities. We are seeking Part Time (16-24 hours per week) team members who are driven by purpose and dedicated to making a lasting impact through person-centered care and supportive services.

Requirements

  • Currently enrolled in a Social Work, Counseling, Mental Health Counseling, or related human services program (Bachelor's or Master's level)
  • Genuine interest in and respect for individuals who have experienced chronic homelessness and complex medical or behavioral health needs
  • Ability to work independently at an off-site partner agency location with minimal day-to-day supervision
  • Strong organizational skills and attention to detail — you thrive managing multi-step processes
  • Clear, empathetic communication skills across a range of audiences (clients, clinical staff, external providers)
  • Discretion and sound judgment when handling sensitive client information

Responsibilities

  • Support clients through all stages of the program enrollment process, working independently at a partnering agency site
  • Collect and track required medical forms from clients' primary care providers (PCPs)
  • Coordinate scheduling and logistics for nursing and care management visits
  • Serve as a consistent, friendly point of contact for clients navigating paperwork and appointments
  • Maintain accurate documentation of enrollment status and outstanding tasks
  • Collaborate closely with the program team to flag barriers and problem-solve in real time
  • Assist with follow-up outreach to clients, PCPs, and partner agencies as needed
