Our mission is to deliver seamless, person-centered support that helps people stabilize, heal, and move forward with confidence. In this role, you will: Conduct screenings and assessments to guide diagnosis, placement, and individualized treatment plans. Provide individual and group counseling using evidence-based, trauma-informed practices. Offer case management that supports clients navigating substance use, mental health, physical health, housing, education, and employment needs. Collaborate closely with River Haven’s clinical and housing teams to ensure cohesive, client-centered care. Participate in case conferences, clinical meetings, and quality assurance activities.
