River Haven is Central City Concern’s newest integrated program, combining housing, healthcare, and employment services to create real pathways out of homelessness. The mission is to deliver seamless, person-centered support that helps people stabilize, heal, and move forward with confidence. In this role, you will conduct screenings and assessments to guide diagnosis, placement, and individualized treatment plans. You will provide individual and group counseling using evidence-based, trauma-informed practices, and offer case management to support clients navigating substance use, mental health, physical health, housing, education, and employment needs. The position requires close collaboration with River Haven’s clinical and housing teams to ensure cohesive, client-centered care, and participation in case conferences, clinical meetings, and quality assurance activities.
