About The Position

At Holon Health, we are rebuilding the healthcare system to serve justice-involved individuals living with Substance Use Disorder (SUD). We are seeking an experienced and compassionate Nurse Practitioner licensed in Iowa to provide telehealth-based care, including assessments, medication management, and care coordination, to support clients on their recovery journey. This role involves collaboration with a multidisciplinary team and judicial partners to address clients' mental, emotional, and physical needs.

Requirements

  • Active and unrestricted NP license in Iowa.
  • Active DEA license (and CDS where applicable).
  • Several years of experience in behavioral health, addiction medicine, or psychiatry.
  • Strong documentation skills and proficiency in EHR platforms.
  • Licensed and experienced Nurse Practitioner.
  • Deeply compassionate toward individuals living with SUD and mental health challenges.
  • Experienced in addiction medicine or psychiatry, with a commitment to person-centered care.
  • A strong collaborator who thrives in integrated and team-based care models.
  • Organized, flexible, and capable of navigating complex systems.
  • Clear and confident in virtual communication with both clients and team members.

Nice To Haves

  • Multiple and additional state licenses are a plus.
  • Board certification in specialty preferred.

Responsibilities

  • Conduct health evaluations to assess substance use, co-occurring conditions, and overall functioning.
  • Diagnose health conditions and create individualized treatment plans.
  • Prescribe and manage medications, monitor for side effects, and adjust treatment as needed.
  • Collaborate with the Holon care team and external providers to deliver coordinated, whole-person care.
  • Serve as a clinical consultant on complex or high-need cases.
  • Educate clients and families on treatment plans, medications, and recovery support strategies.
  • Maintain accurate and timely documentation in compliance with ethical, legal, and medical standards.
  • Participate in team meetings, professional development, and continuous training.
