About The Position

New Paths, Inc. is looking to fill a full-time, non-exempt position for an SUD Home Health worker. The SUD Home Health Coordinator works with clients as an advocate and acts as a liaison in providing appropriate community resources. The SUD HH will identify and gather community resources, observe/participate in client assessments and action plan.

Requirements

Excellent interpersonal and customer service skills.
Ability to interact professionally and respectfully with staff and clients.
Excellent verbal and written communication skills.
Proficient in Microsoft Office Suite or related software.
Ability to work independently.
Ability to coordinate and effect a smooth flow of behavioral-based information and data
Skill in communicating with a variety of people from varying backgrounds
Excellent organizational skills, including the ability to deliver information and services in a timely fashion. Time management.
Exhibit problem solving and conflict resolution techniques.
Minimum of 3 years’ experience in health services field required, SUD experience preferred.
CCAR or MDHHS peer recovery coach certification required.
High School Diploma or GED

Nice To Haves

SUD experience preferred.
CHW Certification, preferred.

Responsibilities

Support and adhere to the NPI Code of Ethics, work rules, policies, plans and procedures to promote a safe and secure environment for the residents, visitors and employees of New Paths.
Identify and gather appropriate community resources, including but not limited to mental health, SUD, medical (health education), housing, financial resources.
Provide problem-solving techniques with clients.
Meet with and communicate with clients regularly.
Participate in health promotion activities or health education class
Observe/participate in client assessments.
Assist in implementing client action plans.
Develop and maintain education materials as resources for staff and clients.
Develop and maintain cooperative working relationships with agencies and organizations in the community.
Document activities and record information.
Other duties as assigned.
