About The Position

Southeast is seeking a mission driven individual to help patients achieve long-term recovery and improve their overall well-being. We are seeking a dedicated and skilled Substance Use Disorder Case Manager to join our team and make a meaningful impact on the lives of those we serve. Your role will include assessing patient needs, developing personalized treatment plans, coordinating services, and advocating for patients to ensure they receive comprehensive support throughout their recovery journey.

Requirements

  • Bachelor’s degree in social work, Psychology, Counseling, or a related field.
  • Certification or licensure in substance use counseling (e.g., CDCA, LCDC, etc.)
  • Experience working with individuals affected by substance use disorders.
  • Strong knowledge of addiction treatment modalities and community resources.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Compassionate and non-judgmental attitude toward individuals facing substance use challenges.
  • Valid driver's license and insurance.
  • As a healthcare provider, Southeast Healthcare is a non-smoking employer.

Responsibilities

  • Conduct initial assessments to determine patient needs and create individualized treatment plans.
  • Provide case management services to patients, ensuring they receive appropriate care and resources.
  • Advocate for patients and collaborate with treatment teams, healthcare providers, and community agencies.
  • Monitor patient progress and provide ongoing support.
  • Help patients access resources such as housing, employment, and social services.
  • Educate patients about substance use disorders and available treatment options.
  • Document and maintain accurate progress notes.
  • Offer emotional support and encourage patients to build coping strategies for lasting recovery.
  • Facilitate group or individual counseling sessions, if required.
  • Stay updated on the latest research and best practices in the field of substance use disorder treatment.
