Success Coach is responsible for implementing and conducting case management, services, and training for participants. Meet regularly (in-person or virtually) with participants to assess program progress, provide support, and offer constant encouragement. Provide meaningful coaching opportunities to help program participants develop problem-solving skills, life skills, and learn to set priorities and identify their values. Provides career navigation, coaching, and stabilization resources to facilitate positive outcomes for participants seeking to advance in career opportunities. Responsible for delivering holistic and comprehensive strength-based case management. Complete a comprehensive strength discovery assessment that provides quality information about each participant’s strengths, family supports, academic challenges, risks, and barriers to success, covering multiple domains, e.g., individual, family, school, and community. Provide support services to enhance program participants' successes, such as community referrals, sending employment opportunities, and job retention coaching. Conduct individualized goal development, long-term plans, and progress monitoring. Meet with each participant weekly or bi-weekly based on needs identified in the assessment. Meet with parents/caregivers to establish rapport and then, as needed, to provide support and connection to services. Be available evenings and weekends to meet with participants and families. As needed. Support participant in creating clearly stated academic, training, and personal goals that address risk factors identified during the assessment process, which may include review of the results of the PYDI assessment tool. Document the progress participants make toward meeting their goals and work with the participant to develop new goals as needed. Collaborate with community partners to address challenges, advocate on behalf of participants, and connect to services such as training. Monitors and evaluates participants’ progress; provides guidance and coaching to help participants resolve issues and remain motivated to obtain and maintain employment. Ensure participant information is timely entered into the appropriate form, database, or report and maintain confidentiality requirements. Comply with the agency’s policy and procedures and Commission on Accreditation of Rehabilitation Facilities standards. Performs other duties as assigned.
