About The Position

The Program of Assertive Community Treatment (ACT) program provides individualized, comprehensive care to individuals with schizophrenia, schizoaffective disorder, and bipolar I and living within the community. The program is an alternative to traditional outpatient treatment that was designed to mimic hospital-level wraparound support to help individuals maintain in the community, rather than bounce back and forth from the hospital. Our goal is to help people retain their independence in the community and help them live a life that is not defined by their illness. The outreach is assertive, and the team is highly integrated and multidisciplinary. We meet individuals where they are at and bring our support to them by going to settings and spaces where they feel safe and comfortable. The Substance Use Specialist provides integrated dual disorder treatment to individuals served by the Assertive Community Treatment (ACT) team who are experiencing substance use and co-occurring mental health challenges. This role is responsible for delivering evidence-based, person-centered services that support recovery and promote long-term stability.

Requirements

  • LADC II, LADC I, LMHC, LCSW or LICSW, required.
  • Valid driver’s license and reliable transportation, required.

Nice To Haves

  • Previous experience and training in integrated dual disorders treatment, highly preferred.

Responsibilities

  • Conduct comprehensive substance use assessments that evaluate the interaction between substance use and mental health conditions
  • Assess and monitor individuals’ stages of change and treatment progress, adapting interventions accordingly
  • Utilize outreach and engagement strategies, including motivational interviewing, to support participation in treatment
  • Deliver evidence-based interventions, including cognitive behavioral strategies and relapse prevention planning
  • Provide stage-matched treatment approaches aligned with each individual’s readiness for change
