As a member of the designated community Mental Health & Substance Use Services or Integrated Response team, this role supports engagement, care coordination, and system navigation across substance use & mental health services, local service areas, and regional and provincial services. The emphasis is on reducing barriers to entering or re-entering the system of care and facilitating timely treatment and rapid re-engagement of clients. This position provides client and family-centered care through a trauma-informed approach and evidence-informed practice to individuals and families affected by serious and persistent mental disorders and alcohol and drug addictions. As a clinician in a shared-team model, responsibilities include initial screening, intake, client assessment, treatment recommendations and plans, determining referral needs, follow-up support, group education, liaison with community resources, counseling clients, and outreach. For those assigned to Aftercare services, the role involves being the "most responsible clinician" for specific clients, depending on their primary needs. The role requires maintaining respect for client dignity and promoting maximum client self-determination and independence. Participation in local committees and internal/external case conferences is expected. The role may involve developing and delivering in-services, presentations, and training. Process improvement activities and adherence to clinical and customer best practices are also part of the role. There may be a requirement to work at multiple locations or on an outreach basis to assess and engage with clients in their own environment and community.
