About The Position

Supports Cherry Creek School District students and families by increasing access to public health insurance programs and related community resources. The role provides outreach, education, and navigation support to help families understand eligibility, complete applications, and connect with appropriate services. The position also strengthens districtwide efforts to identify uninsured or under-resourced students, coordinates with internal and external partners, and contributes to initiatives that reduce barriers to health coverage and essential supports.

Requirements

  • Criminal background check and fingerprinting required
  • Bachelor's degree or equivalent experience
  • Two (2) years of professional work experience in healthcare, benefits, or administrative assistant/clerical experience
  • Colorado driver’s license
  • Intermediate knowledge of Microsoft Office, and Google Suite
  • Advanced knowledge of office equipment such as phones, copiers, fax machines, email, etc.
  • Advanced knowledge of community and school-based programs that support student health care resources and access along with other basic life needs such as food security
  • Exceptional problem-solving and project management skills
  • Exceptional conflict resolution, motivation, and working leadership skills
  • Exceptional verbal and written communication and interpersonal relations skills
  • Ability to work independently and collaboratively with others

Responsibilities

  • Provide outreach, education, and individualized support to families regarding Health First Colorado, CHP+, and other public health insurance programs, including assistance accessing services in person, by phone, and at school or community locations.
  • Assist families with completing applications for Medicaid, CHP+, and other low- or no-cost health plans and provide referrals to community partners when families are not eligible for state programs.
  • Support families in completing applications for additional programs that address health care access, preventative care, basic needs, and community resources.
  • Access authorized state systems (e.g., Health First Colorado portal) to verify insurance coverage, review eligibility information, and obtain data needed to support families.
  • Respond to referrals from district departments and families requesting information about public health programs, and initiate contact with families who have requested assistance.
  • Develop, implement, and distribute outreach and enrollment materials, and participate in school, district, and community events to increase awareness of health coverage options and district resources.
  • Maintain accurate records and documentation related to outreach, enrollment, and services provided, and enter required program data into district systems.
  • Provide ongoing follow-up and case management support to ensure families maintain access to health care, insurance coverage, preventative care, and community resources.
  • Build and maintain partnerships with community organizations and agencies that enhance family access to health care, basic needs support, and resource navigation, and collaborate with district departments to identify uninsured or underinsured students.
  • Perform other duties as assigned or requested.
