About The Position

The Structural Heart Nurse Coordinator is the primary point of contact for patients and their families, managing the process from initial contact through telephone triage and screening. This role follows the patient throughout their continuum of care, including inpatient admission and follow-up at 30 days and one year. The coordinator ensures continuity of care through optimized patient assessment, clinical triage coordination, direct communication with physicians, and daily patient management. The Structural Heart Nurse Navigator collaborates with interventional cardiologists, cardiothoracic surgeons, and clinical staff across multiple departments to coordinate the care of patients with structural heart disease, including those with aortic, mitral, and tricuspid valve disease undergoing evaluation or selection for Transcatheter Aortic Valve Replacement (TAVR), Mitral Transcatheter Edge-to-Edge Repair (M-TEER), and Tricuspid Transcatheter Edge-to-Edge Repair (T-TEER), throughout their pre- and post-procedure care. This role also participates in developing marketing strategies, screening programs, and other opportunities for program development. The coordinator ensures positive customer relationships by responding promptly to needs, managing difficult situations professionally, seeking feedback for service improvement, providing timely assistance, and meeting commitments. Key responsibilities include obtaining STS risk score, Thoracic and Cardiovascular (TCV) in-hospital mortality score, and performing frailty, functional, and quality of life testing. Accurate and thorough documentation is essential to meet coding, STS registry requirements, certification, accreditation, and regulatory requirements (e.g., CMS requirements in National Coverage Decision for Transcatheter Aortic Valve Replacement (TAVR)). The role initiates and drives performance improvement activities, working closely with the STS data analyst to ensure TVT registry data integrity and monitor patient processes and outcomes. Case studies for morbidity and mortality reviews are prepared, and staff professional development is supported to ensure Structural Heart Disease educational content for orientation and annual requirements remains current. Oversight of the Structural Heart Disease schedule and processes is managed in collaboration with procedural areas, IP areas, anesthesia, and appropriate personnel. The coordinator collaborates with healthcare team members to coordinate patient care.

Requirements

Bachelor’s degree in nursing from an accredited school.
Minimum of three years of experience.
Experience in Cardiology and/or Intensive Care required.
Previous leadership and experience with projects.
Basic Life Support certification via the American Heart Association within 90 days of hire.
