About The Position

As a Street Outreach & Youth Case Manager, you'll build trusting relationships with youth and young adults by meeting them where they are—both physically and emotionally. Through outreach, advocacy, case management, and connection to essential resources, you'll help young people identify goals, overcome barriers, and move toward greater stability and independence. This role blends outreach, relationship-building, crisis response, and case management while working collaboratively with community partners, peer outreach staff, and the Youth Resource Center team to create meaningful opportunities for young people. Every interaction is an opportunity to build trust, restore hope, and help someone take the next step forward.

Requirements

  • High school diploma or equivalent required.
  • Strong communication, relationship-building, and organizational skills.
  • Ability to work independently in the community while collaborating effectively with a multidisciplinary team.
  • Self-motivated, adaptable, creative, and passionate about supporting young people.
  • Successful criminal background check required. Previous legal or carceral involvement does not automatically disqualify applicants.

Nice To Haves

  • One year of related experience working with youth, young adults, or vulnerable populations preferred.
  • Lived experience is welcomed and highly valued.
  • Experience in outreach, homelessness services, behavioral health, sexual health education, or community engagement is a plus.

Responsibilities

  • Engage youth and young adults in the community using a compassionate, low-barrier, relationship-centered approach.
  • Meet young people where they are and provide consistent, respectful support without judgment.
  • Build authentic relationships that encourage engagement and long-term connection.
  • Support youth in identifying goals and developing individualized plans for stability and independence.
  • Connect youth with housing, healthcare, education, employment, behavioral health, and other community resources.
  • Provide ongoing case management and advocacy as trust develops.
  • Support family engagement and mediation when reunification aligns with a young person's goals.
  • Distribute survival supplies and basic needs items that promote health and safety.
  • Help youth access food, hygiene supplies, clothing, transportation, and emergency resources.
  • Respond to crises with empathy while promoting safety and informed decision-making.
  • Partner with Youth Resource Center staff to provide pro-social activities and community engagement opportunities.
  • Encourage healthy relationships, permanent connections, educational success, employment, and safe housing.
  • Work alongside Peer Outreach Staff during community outreach and engagement activities.
  • Build and maintain strong relationships with schools, healthcare providers, shelters, behavioral health organizations, and youth-serving agencies.
  • Participate in team meetings, case consultations, and ongoing professional development.
  • Maintain accurate documentation and support program reporting requirements.
