About The Position

Change Grow Live is a charity dedicated to making a difference in the lives of their Service Users by offering support and respect in a safe environment. This role is for a compassionate and proactive individual to join the team in Nottinghamshire as a Street Outreach Recovery Co-ordinator, dedicated to supporting people who are rough sleeping or at risk of homelessness. The role involves working across nights and weekends, delivering street outreach and night shelter support to help individuals make positive changes and move towards stable accommodation. It is a hands-on role that involves engaging with people at all times of day and night, building trusting relationships, and empowering them to express their strengths and aspirations while taking steps towards a more settled future. The position is located in Nottinghamshire and requires cover for the whole county. The hours are 37.5 per week, and the contract type is Fixed Term until March 31st, 2028. Full-time hours at Change Grow Live are 37.5 hours per week, with part-time roles having pro-rata salary and payments based on contracted hours.

Requirements

  • Experience of engaging and providing meaningful support to individuals with severe multiple disadvantages.
  • Experience of communicating effectively with people requiring support.
  • Experience of recording relevant sensitive information in a timely manner.
  • Knowledge of drugs and alcohol support.
  • Knowledge of at least one of the following: housing; homelessness; the benefits system, mental or physical health, the criminal justice system, domestic abuse.
  • Experience of supporting people who may be affected by homelessness, mental health and substance use.
  • Knowledge of Safeguarding Procedures and their application.
  • Experience of working with people in crisis.
  • Knowledge and experience of delivering trauma-informed support.
  • Ability to work as part of a multi-disciplinary team.
  • Excellent Interpersonal Skills / Ability to build rapport.
  • Ability to work independently; adhering to local risk management policies.
  • Ability to problem-solve in challenging situations.
  • Service user focused, and service user led in support planning.
  • Ability to walk for several hours each day to complete outreach activity.
  • Ability to manage individuals that have multiple unmet needs and take support to the individual.
  • Full driving licence.
  • Willingness to drive the Outreach Van.
  • Must already have the right to work in the UK.
  • Role is not eligible for visa sponsorship.

Responsibilities

  • Working as part of the Nottinghamshire county street outreach team.
  • Supporting people in the community who have experience of rough sleeping/homelessness and are using drugs and/or alcohol.
  • Delivering street outreach and night shelter support.

  • Covering early morning and evening outreach shifts.
  • Driving the health bus.
  • Engaging assertively with individuals who services may have not been able to reach in the past.
  • Recording information about activities undertaken.
  • Working in partnership with other providers.
