About The Position

Camillus Health Concern is seeking a compassionate and dedicated RN or LPN to join our Lazarus Project as a Street Medicine Outreach Project Coordinator. This is a unique opportunity to take your skills beyond the traditional healthcare setting and make a meaningful impact in the lives of individuals experiencing homelessness and other vulnerable populations throughout our community. As part of the Lazarus Project, you will work alongside a multidisciplinary team to bring healthcare, outreach, and supportive services directly to individuals who face significant barriers to accessing care. Requirements

  • Active FL Licensed RN or LPN with at least 2-3 years of experience.
  • Bilingual (English/Spanish), Creole a plus
  • Computer literate; previous experience with Electronic Medical Records (EPIC).
  • Health education experience.
  • Community and public health experience.
  • Culturally sensitive to the social, psychological, and healthcare needs of underserved populations.
  • Commitment to the Mission and Values of the Hospitaller Order of St. John of God.

Nice To Haves

  • Experience coordinating community programs or multidisciplinary teams preferred.
  • Experience in an FQHC, community health center, street medicine program, homeless health program, or other safety-net organization preferred.
  • Experience developing community partnerships preferred.
  • Knowledge of Miami-Dade County community resources and HMIS (Homeless Management Information System - countywide information system used to document and coordinate services for people experiencing homelessness or who are at risk of homelessness)

Responsibilities

  • Collaborate and work closely with the LPL to provide day-to-day leadership and coordination of the Lazarus Project.
  • Support onboarding and orientation of new Lazarus Project staff and identify training needs.
  • Coordinate schedules, locations, routes, and team assignments and conduct regular team huddles and or meetings.
  • Ensure that outreach efforts are directed toward individuals and communities with significant barriers to health care. Identify operational barriers and develop practical solutions.
  • Enter and track data in the HMIS system accurately, consistently, and in a timely manner.
  • Serve as the primary point of contact for Lazarus Project coordination.
  • Regularly communicate program activities, challenges, accomplishments, and needs to organizational leadership.
  • Assist the LPL with establishing annual goals, objectives, and priorities for the Lazarus Project.
  • Meet individuals where they are and provide a welcoming pathway into health care and supportive services and develop relationships with individuals experiencing homelessness through consistent and respectful engagement and maintain professional boundaries.
  • Coordinate appropriate referrals and follow-up.
  • Maintain a visible and consistent presence within the community.
  • Promote trust and continuity by ensuring that outreach commitments are followed through.
