About The Position

Summit Home Care and Hospice is searching for an experienced and motivated STNA to join our Home Health & Hospice teams. At Summit, we believe healthcare should be personal, compassionate, and delivered where it matters most - at home. This role involves working under the supervision of a Registered Nurse or therapist to perform various personal care services, observe patients, and document care provided. The STNA will be assigned by a Registered Nurse to ensure quality, continuity, and safety of patient care. The position is primarily on the road for patient visits within a ~30-mile radius, with opportunities for training at the local office or remotely. Mileage is reimbursed.

Requirements

  • Must meet the training and licenses requirements in accordance with state and federal laws of STNA.
  • Must be at least 18 years of age.
  • Must be able to read and follow written instructions and document care that is given.
  • Self-directing with the ability to work with little direct supervision.
  • Empathy for the needs of the ill, injured, frail and impaired.
  • Demonstrates tact, patience, and good personal hygiene.

Responsibilities

  • Perform various personal care services as necessary to meet the patient's needs.
  • Observe patients, report these observations, and document observations and care performed.
  • Provide personal care including, but not limited to: bathing and/or shampooing hair, assisting with dressing and undressing, helping with toileting activities, assisting with oral hygiene, administering skin care to prevent breakdown and back rubs/massages as necessary.
  • Change bed linens and assist with other laundry as appropriate.
  • Maintain a clean and orderly living area for patients, as needed.
  • Plan and prepare nutritious meals and assist in feeding the patient if necessary.
  • Take and record oral, rectal, and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills of competency).
  • Assist in ambulation and exercise according to the plan of care.
  • Perform range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
  • Assist patients in the self-administration of medication.
  • Meet the safety needs of patients and use equipment safely and properly (foot stools, side rails, etc.).
  • Report on patient's condition and significant changes to the assigned nurse.
  • Adhere to the organization's documentation and care procedures and standards of personal and professional conduct.
  • May care for patients in home care and hospice departments as the census/caseload allows.
