About The Position

The Care Team Hospice is looking for a Full-Time STNA for Dover, Ohio and surrounding areas. Primary hours for this position are Monday - Friday 8:30am - 4:30pm. As a CNA, under the direction and ongoing supervision of the registered nurse, you will provide essential personal care assistance to the patient, offering emotional support and compassionate care to the patient and family unit.

Requirements

  • Licensed or registered in the state in which currently practices.
  • Must pass a criminal background check & MVR check.
  • Completed health statements acknowledging the ability to perform the duties of the position.
  • Valid state drivers license.
  • Proof of current automobile insurance.
  • TB testing per agency policy.
  • High School Diploma or GED.
  • Graduate from an approved training and competency course.
  • Maintains current certification as required by the State.
  • Maintains current knowledge and practice of infectious disease protocols.

Responsibilities

  • Provide essential personal care assistance to the patient: bath, care of mouth, skin, and hair.
  • Assist a patient to the bathroom or in using bedpan or commode.
  • Assist with bed transfers as well as ambulation activities as ordered under the care plan.
  • Perform range of motion exercises which the Aide has been taught by appropriate professional personnel.
  • Perform such incidental household services as are essential to the patient's well-being at home.
  • Prepare meals including a special diet.
  • Take a patient's temperature, pulse, respiration, and blood pressure if assigned by a registered nurse.
  • Assist with shopping if the patient has no other resources available.
  • Provide emotional support and compassionate care to the patient and family unit.
  • Demonstrate listening skills.
  • Offer encouragement when appropriate.
  • Practice patience in voice and manner.
  • Maintain required competency and skills.
  • Document initial and ongoing every 14-day supervision and evaluation by RN on each case assignment.
  • Complete a clinical note for each visit made to a patient's home (including a private residence, assisted living facility, group homes, or skilled nursing facility) within 48 hours of the visit.
  • Participate in an annual performance review.
  • Report any changes in the patient's condition or family situation immediately by calling the Hospice office and notifying the RN.
  • Practice Standard Precautions according to OSHA regulations and policy.
  • Adhere to all policies and procedural guidelines.
  • Attend at least twelve (12) hours of in-service education programs per year.
  • Maintain current certification as required by the State.
  • Maintain current knowledge and practice of infectious disease protocols.
  • Maintain the agency's mission, philosophy, and core values.
  • Ensure compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management.
  • Always maintain patient confidentiality, including all HIPAA regulations.
