About The Position

Glen Oaks Hospital is a 54-bed private mental health hospital in a relaxed setting in Greenville, Texas. For over 30 years, Glen Oaks Hospital has been a leader in the field of mental health and chemical dependency care, and our staff is dedicated to researching the latest treatment approaches and applying this to our overall treatment philosophy. The Step-Down Navigator is responsible for implementing a revised process of meeting patient needs for a continuum of care. They will meet with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes. They will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed. It is expected that the Step-Down Navigator will identify implementation barriers and evaluate the new process with the facility leaders using the data. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred. Hours: Day shift, Monday through Friday, 8am to 5pm.

Requirements

  • Bachelor's degree in mental health or behavioral sciences field, preferred.
  • Two years minimum experience in psychiatric / mental health field, preferred.
  • CPR BLS certification and successful completion of Handle with Care (HWC) and Verbal De-escalation (VDE) training.
