Start of Care (SOC) Nurse (Ocala, FL)
About The Position
Home Heart Florida is a pediatric home health agency supporting children with complex medical needs at home. The Start of Care Nurse conducts initial home evaluations, establishes individualized care plans, and helps families begin a safe, high-quality home health experience. The role serves as an important point of contact for families during the start of their care journey and covers Marion County in the Ocala, Florida region. Visit types include Start of Care, Resumption of Care, Recertification, and FHHA Supervisory Visits. The position is part-time or per diem and requires an active Florida RN license.
Requirements
- Active Florida RN license and BLS certification.
- At least 1 year of experience completing Start of Care visits in home health.
- Strong clinical documentation and EMR experience.
- Valid driver’s license and reliable transportation, with willingness to travel to patient homes.
- Comfort working with medically complex pediatric patients.
Nice To Haves
- Bilingual English/Spanish; especially relevant for Miami-Dade and Broward.
- Experience with Statewise EMR.
- Pediatric home health or private duty nursing experience.
Responsibilities
- Conduct comprehensive Start of Care assessments in patients’ homes.
- Develop and initiate individualized care plans based on physician orders.
- Educate family caregivers about care needs, equipment such as vents, G-tubes, and pumps, medications, and safety.
- Complete thorough and accurate clinical documentation.
- Communicate assessment findings and care plans to the clinical team and coordinating nurse.
