The Stars Population Health Strategy Lead develops and executes population health strategies that improve outcomes for defined member populations. This role identifies opportunities through data analysis, designs evidence-based initiatives, monitors performance, and partners with internal and external stakeholders to address gaps in care, reduce disparities, and support quality improvement. The ideal candidate is a strategic thinker with strong analytical, communication, and program management skills. They should be comfortable translating complex data into actionable insights, influencing cross-functional partners, and presenting clear recommendations to leadership. Health Quality and Stars is focused on improving health outcomes and enhancing the care experience for members and provider partners through quality-driven solutions. The organization supports high plan quality as measured by the Centers for Medicare & Medicaid Services through the Five-Star Quality Rating System. The Five-Star Quality Rating System evaluates Medicare Advantage and Prescription Drug Plans across measures such as preventive screenings, chronic condition management, health outcomes, patient experience, and plan operations. The Stars Population Health Strategy Lead will be responsible for developing population health strategies, implementing targeted initiatives, monitoring performance, and partnering with internal and external stakeholders to improve outcomes for priority populations.
