About The Position

Harbor Health is seeking Family or Internal Medicine and Pediatric Physicians to join our Per Diem Staff at our community health center locations. We have all shifts available, including evening and Saturday shifts. The Per Diem Physician provides care, including routine health maintenance, management of chronic conditions, and management of acute illness for patients based upon a per diem schedule arranged by the Site Medical Director.

Requirements

  • Completed accredited physician residency in internal medicine, pediatrics, or family practice as appropriate
  • Current Massachusetts MD, DO license, Board Certified or Board Eligible preferred
  • Current DEA Certification and Mass Controlled Substance Registration
  • Current BLS (Basic Life Support for Health Care Providers) Certification
  • Proficiency in Office 365 including Word, Excel, Outlook and Teams, and Electronic Medical Record (EPIC/OCHIN preferred)

  • Experience with a community health center population and practice setting preferred

Responsibilities

  • Provides primary care coverage and care coordination to those patients who have primary care providers at the health center location.
  • Provides primary care to include but is not limited to outpatient chronic care and acute care visits, routine health maintenance visits as well as outpatient telehealth visits.
  • Provides primary care coordination to include but is not limited to referral for specialty care, coordination of hospital follow-up visits.
  • While scheduled in clinic, manages laboratory results that become available during the hours of their scheduled shift.
  • While scheduled in clinic, supports in-basket coverage for PCPs not on site.
  • Acts as a rapid response MD available if another PCP or team anchor is not available.
  • Supervises mid-level practitioners who are working alongside the per diem provider in clinic.
  • Supports patients in managed care plans and assists patient understanding of such plans, including attention to quality improvement care gaps.
  • Complies with state continuing education laws.
  • Provides credentialing information on a timely basis.
  • Appropriately documents clinical care in the Electronic Medical Record (EMR).
