About The Position
The Staff Pharmacist provides pharmaceutical care services to patients, including oversight, monitoring, preparation, and dispensing of medications, as well as proper documentation of patient medication therapy. The role also provides drug and patient information and oversees information and dispensing systems. The pharmacist ensures compliance with applicable federal, state, and local laws and regulations and may advise physicians on medication utilization. The position is per diem with a rotating shift at St. Mary's Medical Center, a 420-bed acute care hospital in West Palm Beach and part of the Palm Beach Health Network.
Requirements
- Pharm.D. or bachelor of science in Pharmacy.
- Current registered pharmacist license in the State of Florida.
- BLS certification within 30 days of hire.
- ACLS and/or PALS certification if code attendance is required.
Nice To Haves
- Hospital experience.
- Board certification in Pharmacotherapy (BCPS), CGP, or similar.
Responsibilities
- Review and monitor patients’ medication therapy and orders for safety and efficacy, considering each patient’s condition and health status.
- Advise the patient care team on patients’ medication therapy and document interventions.
- Verify orders and review medication profiles for issues including duplication, drug-drug and drug-food interactions, allergies, medication appropriateness, dosing, and frequency.
- Prepare, label, and dispense medications in accordance with physicians’ prescriptions.
- Ensure compliance with federal, state, and local laws and regulations related to dispensing drugs and narcotics and maintaining required records.
- Provide drug and patient information and oversee information and dispensing systems.
- May advise physicians on medication utilization.
