About The Position

IHCS provides an Integrated Delivery System in the home setting, which includes, DME, Respiratory, Home Health and Home Infusion services. IHCS has a select network of Medicare and/or Medicaid Certified and Accredited providers to respond to the needs of our patients – 24/7. The Pharmacist is responsible for pharmaceutical care, dispensing, and monitoring to provide clinical services to home bound or facility-based patients with intravenous infusion, specialty, and injectable medication. This role involves identifying and accurately processing orders for appropriate infusion systems and IV supplies for a given therapy, and collecting and organizing all patient-specific information needed to determine the suitability of patients for home care. Requirements

  • Graduation from an accredited school of pharmacy
  • Current valid Florida License in good standing with the State Board of Pharmacy
  • 2 years' experience in aseptic technique and intravenous solution compounding per USP <797>
  • Homecare experience
  • Ability to fulfill on-call obligations and Saturday rotation

Responsibilities

  • Responsible for pharmaceutical care, dispensing, and monitoring to provide clinical services to home bound or facility-based patients with intravenous infusion, specialty, and injectable medication.
  • Identifies and accurately processes orders for appropriate infusion systems and IV supplies for a given therapy.
  • Collects and organizes all patient-specific information needed by the home care pharmacist to determine the suitability of patients for home care.
  • Participates in achievement of monthly, quarterly and yearly projections for operations, inventory control, quality assurance, inspectors/audit and staff.
  • Remains proficient in all areas of pharmacy practice with continuous review of literature and maintaining current files and reference materials.
