This per diem Staff Pharmacist position involves various shifts and requires weekend and holiday availability. The role focuses on accurate medication order entry, profile maintenance, drug therapy monitoring, and ensuring the safe and effective distribution of medications. The pharmacist will also educate patients and clinicians, collect data for drug use review programs, prepare chemotherapy, and assist with medication reconciliation. This position plays a key role in promoting appropriate antimicrobial use and ensuring patient safety in medication management.

Requirements

B.S. in Pharmacy
Clinical background to help develop programs
Current MA licensure
Good communication skills to relate with medical and nursing staff

Responsibilities

Enters all medication orders correctly into patient medication profiles in compliance with hospital formulary.
Interprets medication orders accurately.
Maintains accurate and complete patient medication profiles.
Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
Consistently does DUR before entering medication orders.
Demonstrates knowledge of appropriate standards, policies, protocols, procedures and practices in a manner that demonstrates adequate baseline knowledge.
Proficient in dosing/monitoring of medications with an established protocol.
Provides double checks as needed on pharmacokinetic consults to detect calculation errors and assess outcome goals.
Documents interventions in the medical record that advocate IV to PO conversion, dose optimization, dose adjustments, therapeutic duplications, drug-drug interactions, allergy conflicts and other interventions groups for Antimicrobial Stewardship and other medication therapies.
Supervises the manufacture and distribution of medications and IV solutions by the Pharmacy Technicians to ensure medications are delivered to the patient care area staff at the scheduled times of administration.
Contacts prescriber about questionable orders and provides information on the correct therapy, noting formulary alternatives when indicated.
Educates patients and clinicians regarding medications.
Collects data for DUR and ADR programs.
Prepares chemotherapeutic medications according to the program so that patients receive the correct dose and medication at the scheduled time.
Systematically gathers and evaluates relevant information and data from the patient interview and/or all the patient information sources.
Consistently looks at Med Recs for appropriate dosing.
Able to recommend appropriate antibiotic therapy based on culture and susceptibility data or suspected infection.
Uses appropriate antibiogram to see sensitivity data.
Demonstrates an understanding and completes all job functions in accordance with the five patient rights.
Follows hospital policy in reporting medication errors and adverse drug reactions.
Assists physicians and nurses with medication reconciliation.
Follows all policies and procedures related to inventory management and control.
Promotes critical pathways that encourage the selection of antimicrobials and antimicrobial combinations that delay or prevent the emergence of resistance.
Follows practices that encourage selection of the antibiotic regimens with the narrowest spectrum of activity based on culture and sensitivity results.
