Requirements

  • Bachelor's degree in pharmacy.
  • Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
  • Current, full, active, and unrestricted license to practice as a pharmacist.
  • A minimum of one (1) year of inpatient pharmacy experience within the past two (2) years post-graduation.

Responsibilities

  • Works collaboratively with multidisciplinary teams to optimize drug therapy.
  • Determines type of substance, strength, dosage, and quantity to be dispensed in accordance with accepted practice and pharmacy standing operating procedures (SOPs).
  • Receives and reviews prescriptions for the full range of pharmaceuticals to include controlled drugs on the formulary.
  • Performs advanced pharmaceutical calculations in the manufacture of intravenous admixtures and complex parental nutrition solutions dealing with body surface area and creatinine clearance rate formulas.
  • Obtains the pharmaceutical, prepares package labels, visually inspects the medication in the bottle to be dispensed, and verifies appropriate medication, checks the printed label affixed to the container against the original prescription for correctness.
  • Reviews and interprets medication orders to inpatients to ensure dosage, route, frequency, potential interactions, and allergies have been screened.
  • Performs medication distributive functions, including, but not limited to, preparation and/or verification of medications, labeling, compounding, and dispensing.
  • Follows established procedures and protocols concerning quality assurance monitoring, security of controlled substances, and disposal of hazardous medications.
  • Participates in orientation, instruction and training of pharmacy technicians, residents, pharmacists, and other health professionals.
  • May be identified per shift as a Code Responder and will respond to Code Blues during a particular shift.
  • All pharmacists can rotate between any ambulatory pharmacy or inpatient pharmacy sites to accommodate staffing needs.
