About The Position

Provides advanced direct medication distribution, clinical leadership, and specialized pharmacy services across assigned inpatient and outpatient service areas. Serves as a subject matter expert within an area of board certification while leading quality improvement initiatives, clinical optimization projects, and best practice development at the departmental level. Partners with interdisciplinary teams to advance patient safety, antimicrobial stewardship, transitions of care, and evidence-based pharmacy practice. Mentors and supports the development of Staff Pharmacists to promote clinical excellence and professional growth.

Requirements

  • Bachelor of Science or Doctorate in Pharmacy (PharmD).
  • Current Montana Pharmacist license or eligibility to obtain upon hire.
  • Current Board Certification (e.g. BCIDP, BCOP, BCPPS, BCACP).
  • Current American Heart Association Basic Life Support (BLS) upon hire.
  • American Heart Association Advance Cardiovascular Life Support (ACLS), or ability to obtain within ninety (90) of hire if working within a setting that requires the certification.

Nice To Haves

  • Postgraduate year one (PGY1) residency training if working within an acute/inpatient setting.
  • Two (2) years of relevant pharmacy practice experience.
  • Experience with Epic/Willow, outpatient pharmacy systems, MPDR/PDMP, and clinical references (e.g., Lexicomp).

Responsibilities

  • Verifies medication orders and prescriptions for indication, dosing, route, interactions, allergies, and duplication to ensure safe and accurate therapy.
  • Performs advanced prospective and concurrent clinical monitoring and provides evidence-based pharmacotherapy recommendations within board-certified specialty areas.
  • Leads antimicrobial stewardship activities, treatment pathway development, and order-set optimization in collaboration with interdisciplinary teams.
  • Provides patient counseling on medications, adverse effects, adherence, device technique, and immunizations as assigned.
  • Coordinates medication reconciliation, discharge counseling, and transitions-of-care services, including support of “med-to-bed” initiatives.
  • Responds to clinical emergencies and alerts by preparing, recommending, and documenting medications per established protocols.
  • Utilizes Epic/EHR, automated dispensing systems, outpatient pharmacy systems, MPDR/PDMP, and clinical references to support safe medication use.
  • Leads or participates in departmental quality improvement, medication safety, and performance improvement initiatives.
  • Develops, evaluates, and maintains clinical protocols, policies, and procedures related to assigned board-certified practice areas.
  • Mentors and supports Staff Pharmacists, residents, and pharmacy technicians through training, case review, and professional development activities.
  • Maintains board certification through completion of continuing education, professional practice requirements, and specialty recertification.
  • Documents clinical interventions, outcomes, and pharmacy activities in the electronic health record and required reporting systems.
