This position involves monitoring, modifying, and evaluating drug therapy for optimal therapeutic outcomes. The pharmacist will interact with patients to obtain drug history, monitor responses, educate, and counsel on current drug therapy and potential side effects. Responsibilities include performing pharmaceutical compounding for sterile and non-sterile preparations, screening prescriptions for accuracy and potential incompatibilities, and advising patients and healthcare staff on dosage, side effects, and storage. The role also involves evaluating medication orders for appropriateness based on clinical guidelines and patient-specific factors, dispensing medications with attention to accuracy and safety, and participating in medication reconciliation and collaborative treatment planning. The pharmacist will provide pharmaceutical care and public health services in various community locations, seek consultation for complex decisions, and use established protocols and guidelines. Individualized patient counseling is a key aspect, as is assisting in quality improvement and medication safety efforts, including adverse drug event tracking and drug utilization reviews. The pharmacist will support documentation for quality assurance and medication error prevention, oversee pharmacy technicians, and perform administrative functions to support the pharmacy department. This includes contributing to policy development, performance improvement, and compliance with accreditation standards. Participation in pharmacy audits, monitoring controlled substance regulations, and supporting staff training are also expected. The role requires completing required training, maintaining licensure and continuing education, and participating in performance reviews. Other duties as assigned by the supervisor are also part of the role.
