About The Position

Under the Department of Pharmaceutical Services, the Pediatric Clinical Staff Pharmacist II provides pharmaceutical care to patients at all acute care pharmacy locations within the UCSF Health System. They may also be asked to participate in pharmaceutical care in other sectors of the Department of Pharmaceutical Services as needed. The Pediatric Clinical Staff Pharmacist II performs a wide range of pharmaceutical care functions to assure that drugs prescribed for and used by patients are safe and effective. These include, for example, obtaining medication histories, reviewing and assessing medication profiles, providing drug information to patients and health care providers, collaborating with clinicians on drugs of choice and therapeutic plans, ensuring access to medications, educating patients on the proper use of medications, and ensuring that patients understand cautions, adverse reactions, and interactions related to their therapy. The Pediatric Clinical Staff Pharmacist II will be part of an interprofessional team consisting of physicians, advanced practice providers, nurses, and other professionals. The Pediatric Clinical Staff Pharmacist II will be based primarily in the inpatient pharmacy setting managing pharmaceutical care for patients of all ages. Depending on the assigned shift and site, the Pediatric Clinical Staff Pharmacist II may also oversee pharmacy technicians to facilitate insurance authorizations for medications, as well as prepare and dispense prescription medications and pharmaceutical products for patient care. The Pediatric Clinical Staff Pharmacist II may also be responsible for the oversight of clinical activities and delivery of services.

Requirements

  • Possession of a license to practice pharmacy in the state.
  • Experience in a clinical pharmacy setting.
  • Ability to obtain medication histories.
  • Ability to review and assess medication profiles.
  • Ability to provide drug information to patients and health care providers.
  • Ability to collaborate with clinicians on drugs of choice and therapeutic plans.
  • Ability to ensure access to medications.

  • Ability to manage pharmaceutical care in an inpatient pharmacy setting.
  • Ability to work as part of an interprofessional team.
  • Understanding of insurance authorizations for medications.
  • Ability to prepare and dispense prescription medications and pharmaceutical products.
  • Ability to oversee clinical activities and delivery of services.
