About The Position

Under the Department of Pharmaceutical Services, the Clinical Staff Pharmacist II provides pharmaceutical care to patients at all acute care pharmacy locations within the UCSF Health System. They may also be asked to participate in pharmaceutical care in other sectors of the Department of Pharmaceutical Services as needed. The Clinical Staff Pharmacist II performs a wide range of pharmaceutical care functions to assure that drugs prescribed for and used by patients are safe and effective. These include, for example, obtaining medication histories, reviewing and assessing medication profiles, providing drug information to patients and health care providers, collaborating with clinicians on drugs of choice and therapeutic plans, ensuring access to medications, educating patients on the proper use of medications, and ensuring that patients understand cautions, adverse reactions, and interactions related to their therapy. The Clinical Staff Pharmacist II will be part of an interprofessional team consisting of physicians, advanced practice providers, nurses, and other professionals. The Clinical Staff Pharmacist II will be based primarily in the inpatient pharmacy setting managing specific patient population if applicable. Depending on the assigned shift and site, the Clinical Staff Pharmacist II may also oversee pharmacy technicians to facilitate insurance authorizations for medications, as well as prepare and dispense prescription medications and pharmaceutical products for patient care. The Clinical Staff Pharmacist II may also be responsible for the oversight of clinical activities and delivery of services.

Requirements

  • Possession of the legal requirements to practice as a Pharmacist in California.
  • Possession of a current California Pharmacist license.
  • Completion of a Doctor of Pharmacy (Pharm.D.) degree or equivalent.
  • Completion of a PGY-1 Pharmacy Practice Residency or equivalent experience.
  • Completion of a PGY-2 Pharmacy Practice Residency in Cardiac/Thoracic Surgery or Heart Transplant or equivalent experience.
  • Board certification in pharmacotherapy or critical care pharmacy or willingness to obtain within two years of hire.

  • Experience in a specialized area such as cardiac/thoracic surgery or heart transplant.

  • Experience in precepting pharmacy residents or students.
