The SWAT Registered Nurse is an autonomous, highly skilled clinician who supports inpatient units through early identification and intervention for deteriorating patients. This role rounds proactively on high risk individuals—such as those with elevated Deterioration Index scores or Sepsis Alerts—and responds to CPR, Rapid Response, and Inpatient Stroke Team activations to enhance patient safety and prevent clinical decline. The SWAT RN delivers expert assessment, coordination, and evaluation of care; provides patient and family education; safely administers medications; and supports unit efficiency through effective resource use. The RN mentors new staff, participates in shared governance and quality improvement, integrates evidence based practice, and may assume Charge Nurse responsibilities when assigned. Through the diagnosis and treatment of human responses and advocacy for individuals and families, the SWAT RN maintains full accountability to the New York State Nurse Practice Act, United Health Services policies, the ANA Scope and Standards of Practice, the ANA Code of Ethics, and the Bill of Patient Rights, promoting a caring, healing, and patient centered environment.
