About The Position

Home Health provides community health care to patients wherever they call home. Services include home health visits for adults with acute and chronic medical conditions.

Requirements

• At least 2 years of professional nursing (RN) home visiting/public health work experience
• Must have a valid driver’s license and the availability of an insured vehicle to travel to patient's homes or agreed upon meeting places
• Graduation from an accredited school of nursing
• Possession of a valid license as a Registered Nurse issued by the State of Minnesota

Nice To Haves

• Public Health/ Home Care professional nursing (RN) experience
• EPIC proficiency and OASIS experience

Responsibilities

• Protecting, promoting wellness and optimizing health
• Triaging patients by assessing severity of symptoms and referral to appropriate setting or level of care
• Alleviating suffering through the treatment of the human response
• Partnering with the patient, their family and significant others
• Documenting interventions and an individualized plan of care
• Educating patient on their disease process
• Assuring continuity of care
• Educating patient and family in preventative care which promotes health and well being
