Overview of Position: The Nurse is an essential member of the patient center medical home team. Responsible for managing the individualized patient care, by promoting and restoring patient’s health through the nursing process; collaborating with provider and multi-disciplinary team members; provides physical and psychological support.

Requirements

  • Diploma in nursing with 2-years’ experience
  • Current licensure through South Carolina State Board of Nursing
  • CPR (Cardiopulmonary Resuscitation) or BLS (Basic Life Support) certification required
  • Possess excellent customer service skills and be well organized.
  • At least 2 years experienced working in a primary or specialty care medical practice.
  • Ability to work in a high-pressure environment.
  • Possess the ability to work with patients, families, clinical, non-clinical staff and providers from a variety of backgrounds and lifestyles while maintaining a non-judgmental attitude.
  • Must possess excellent oral and written communication
  • Must have the ability to collaborate with others to improve quality and address needs
  • Ability to take initiative and handle various task simultaneously
  • Knowledge of policies, procedures, regulations, and protocols
  • Knowledge of data bases (ex: EMR systems)
  • Maintain current medical knowledge to include pharmacology (medication names and usage) knowledge required
  • At least two years of experience in a physician’s office
  • Excellent interpersonal skills

  • Ability to administer a variety of treatments and medications as directed
  • Capable of adapting to computer software

Responsibilities

  • Assess the needs of patients based on data collected through history observation, physical exam, telephone triage, and analysis or diagnostic data
  • Performs direct patient care: triage, assessment, procedures, treatment, patient education, and follow up instructions.
  • Assist patients and providers in obtaining medicinal prior authorizations when required
  • Verifies the patient understands the purpose of the visit.
  • Providers preventative Care information matching the patients age, gender, and health status
  • Maintain knowledge of the principles of the skills needed for practical nursing to provide patient care and treatment
  • Knowledge of examination, diagnostic and treatment room procedures
  • Ability to interpret, adapt, and apply guidelines and procedures
  • Basic Critical Care and Chronic Illness Management
  • Required to participate in training opportunities
  • Helps promote office efficiency and encounter volumes
  • Ability to understand and administer vaccines and immunizations to pediatric and/or adult patients
  • Responsible for daily clinical workflow which includes but not limited to: Referral management, telephone encounters, lab results, medication refills, lab Q&A, compliance with sample medication logs
  • Willingness to learn about AIDS Service Organizations and Community Health Centers
  • Assist in training new nurses to the organization in the standard working procedures of HopeHealth
