As an integral member of the clinical support team, the LPN collaborates with multidisciplinary colleagues, supports culture change initiatives, and serves as a role model and mentor. The LPN is expected to uphold the philosophy and mission of Hebrew SeniorLife, recognize patients’ dignity and choices, and contribute to the unit’s activities and social environment while providing safe, efficient, and high-quality care. The LPN practices within professional nursing standards and an assigned primary nursing model, with RN oversight of assessment and care planning. The position includes collaborating on individualized care plans, documenting care in electronic medical records, coordinating with the multidisciplinary team, delegating and supervising nursing assistant care, and addressing safety and quality concerns. The LPN may be assigned charge nurse responsibilities with RN oversight and may complete other duties or special projects consistent with the position.
