About The Position

The Staff Nurse II is a professional caregiver responsible for the assessment, nursing diagnosis, planning, implementation, and evaluation of patient and family care, adhering to the standards of Seton Medical Center and the Nurse Practice Act. This role involves direct patient care in the Emergency Services department, requiring a broad range of clinical skills and knowledge, including age-specific care, emergency response, and patient education.

Requirements

Graduation from an accredited school of professional nursing.
Minimum of six months Registered Nurse experience.
Six months current ER & triage experience or completion of Seton ER Nurse Training Program.
Active California Registered Nurse licensure.
Basic Life Support certification (BLS) required.
ACLS certification required.
PALS certification required.
NIHSS certification required.
Workplace Violence Prevention De-escalation 8-hr (AB508) certification or CPI certification required.
Diffusing Assaultive Behavior Training.
Knowledgeable of the California Nursing Practice Act.
Able to understand and follow all policies, procedures, guidelines, rules and regulations of Nursing Service, Seton Medical Center, Title XXII.
Excellent communication and customer service skills.

Nice To Haves

Baccalaureate degree in Nursing preferred.
Proficient in Evident computerized documentation system preferred.
NRP certification.

Responsibilities

Assesses, collects, and documents health assessment data, focusing on physical, emotional, spiritual, psychosocial, and educational needs of the patient.
Interviews patients and families to complete Triage information.
Identifies actual and potential patient needs and determines priority.
Identifies barriers to learning.
Completes and documents an assessment after initial triage, focusing on abnormal systems and actual/potential problems, and performs ongoing assessments as needed.
Plans nursing care based on the priority of physical and psychosocial-emotional needs.
Develops realistic short-term goals involving the patient and family.
Plans demonstrate understanding of age-related patient needs.
Negotiates with patient, family, and healthcare team members in planning for education and discharge needs.
Implements the plan of care, including prescribed procedures and treatments.
Organizes and prioritizes care based on individual needs and unit activity.
Assesses for changes in patient’s condition and appropriateness of the plan.
Initiates interventions using appropriate clinical expertise and knowledge.
As Primary Nurse, coordinates healthcare team activities for diagnostic and therapeutic interventions.
Evaluates the effectiveness of nursing care delivered to reach the expected outcome.
Evaluates the plan of care according to the patient’s current condition and changing needs.
Accurately and concisely documents the patient’s status and response to care.
Utilizes professional nursing knowledge and skills to assess physical and psychosocial-emotional needs, barriers to learning, educational/learning needs, discharge needs, age-related needs, and issues related to assault or abuse.
Analyzes patient-related data, such as vital signs, medical history, physical exam findings, and laboratory values, including age-specific criteria, to determine appropriate care.
Utilizes advanced, specialized knowledge and skills to implement interventions for all Emergency Services patients, prioritizing needs and using a patient-centered approach.
Provides age-specific health education and teaching to patients and families, addressing barriers to learning.
Clearly communicates health, treatment, and discharge information, ensuring understanding of all instructions.
Effectively performs procedures such as initiating IV catheters and fluids, medication administration, and dressing changes within scope of practice and policies.
Effectively performs other nursing procedures/skills such as cardiac monitoring, rhythm interpretation, defibrillation, and monitoring of conscious sedation, based on specialty training and scope of practice.
Appropriately recognizes and quickly responds to emergency medical situations, including assessing "A-B-Cs," initiating appropriate emergency treatment (e.g., CPR, O2), and communicating critical information.
Maintains all required credentials and training (CA RN license, BLS, ACLS, PALS, NIHSS, AB508).
Remains current on emergency treatments, practices, and procedures through staff meetings, continuing education, in-services, and professional literature review.
Integrates and applies new knowledge from education and research into clinical practice.
Demonstrates knowledge of and competence in age-related patient care.
Demonstrates knowledge of state and federal regulations, such as Title XXII and COBRA/EMTALA, and follows TJC, CDPH, CMS guidelines.
Advocates for public health and safety through patient and family education.
Consistently completes required documentation of the Emergency Services visit.
Consistently demonstrates caring and compassion through behaviors and interactions, recognizing the inherent worth and dignity of patients, families, staff, and physicians.
Will perform duties of Relief Charge RN if called upon.
Adheres to departmental and medical center policies on scheduling, overtime, sick time, and time off.
Arrives punctually, takes appropriate time for breaks, and uses sick time appropriately.
