About The Position

This role involves providing specialized nursing care within the Wound Care Department, focusing on patient assessment, planning, and implementation of care related to wound management. The LPN will collaborate with RNs and Case Managers to ensure continuity of care, patient education, and adherence to protocols. The position requires a strong understanding of wound assessment, staging, measurement, and documentation, as well as the ability to operate clinical equipment safely and follow infection control procedures. The LPN will also function in intake and discharge roles and maintain professional communication with patients, physicians, and staff.

Requirements

  • Must be licensed in the state of Indiana as a Licensed Practical Nurse.
  • BLS or CPR for Healthcare Providers certification required.
  • A minimum of 2 years of Medical/Surgical or wound care experience required.

Responsibilities

  • Perform follow-up nursing assessment of patient's conditions and report findings to RN.
  • Recognize alterations in the patient's healing progress and verbalize this to other members of the health care team.
  • Organize assessment data for accuracy, completeness, and confidentiality.
  • Record wound assessment data in an orderly fashion and communicate, revise, and verbalize pertinent information to other health care team members.
  • Assess, stage, and measure wounds accurately and in accordance with policy and procedure.
  • Photograph wounds according to policy and procedure.
  • Communicate patient care situations that may require intervention, implement nursing actions identified by Staff RN/Case Manager, and evaluate the patient's responses with Staff RN/Case Manager.
  • Complete patient/family education and discharge plans identified in the patient's plan of care.
  • Collaborate with Staff RN/Case manager to identify the patient's present and potential problems from the assessment.
  • Implement a teaching plan in collaboration with Staff RN/Case Manager.
  • Prioritize problems according to impact on health status.
  • Formulate with Staff RN/Case Manager the desired outcomes that are specific and measurable within a certain time frame and consistent with other health provider's expectations.
  • Assess home health care needs, complete a support system evaluation, and assess patient teaching needs from admission to discharge. Communicate information to Staff RN/Case Manager.
  • Participate in implementing planned changes and activities to improve care.
  • Hold self-accountable for the delivery of quality nursing care.
  • Promote harmonious relationships and favorable attitudes among team members.
  • Implement wound care and HBO protocols when clinically indicated.
  • Individualize care according to age specific population.
  • Function as an intake/discharge nurse during clinic times.
  • Foster continuity of care between the Wound Care Department and caretaker (home, hospital, home health, etc.).
  • Prepare clinic records to enable accurate and complete updates to the physician.
  • Assist others within the clinic.
  • Maintain an open, professional communication line with patients, physicians, and staff.
  • Safely operate all clinical equipment.
  • Utilize appropriate infection control procedures.
  • Maintain skills and knowledge through training and continuing education. Complete mandatory education requirements. Attend staff meetings and in-services.
  • Monitor the condition of equipment and report malfunctions to the Supervisor or Director.
  • Performs other related duties as assigned.

  • Conducts all activities in compliance with applicable laws, regulations, standards, and Johnson Memorial Health policies and procedures including Blood and Body Substance Precautions.
