About The Position

Senior Helpers of Northern Queens is seeking a dedicated and compassionate Field Nurse (RN) to oversee the LPN and Caregiver team. This role involves client assessments and direct client care in the home. The ideal candidate will be independent, possess a friendly personality, and be comfortable working with a diverse clientele (Chinese, Korean, Spanish, Russian, Yiddish, and English speaking). Requirements

  • High school diploma is required.
  • Must be a licensed Registered Nurse (RN).
  • Must meet NY state requirements for RN services.
  • Must have safe and reliable transportation to go to and from the client’s location and/or facilities.
  • Have a vehicle available during assignments in order to assist clients with shopping needs if required.
  • Demonstrated ability to effectively interact with colleagues, clients and supervisors.
  • Ability to exercise good judgment and self-control.
  • Good comprehension and communication skills, with the ability to listen to and understand information and ideas presented through verbal and written communication.
  • Must demonstrate enthusiasm, trustworthiness, personal integrity and honesty.
  • Ability to treat clients with dignity and respect.
  • Ability to communicate with clients and to remain calm under stress.
  • Demonstrated ability to operate safely in the workplace.

  • Self-motivated, with a dedication to keeping up to date technically.
  • All Field Nurses will be required to submit or undergo a full annual physical with MMR (MEASLES/MUMPS/RUBELLA) and PPD/TB TEST(less than a year old) as required by the state.

Nice To Haves

  • At least two years of experience working in home health care.
  • Valid and current state driver’s license, registration and insurance
  • Available to work various shifts, including nights, weekends, and holidays.

Responsibilities

  • Complete necessary documentation for nursing assessments and planning evaluation pertaining to clients.
  • Sign off on nursing assessments completed by Staff LPN.
  • Evaluate clients as necessary to ensure client needs are consistent with the care plan.
  • Utilize professional assessment skills to identify physical, mental, and psychosocial needs of clients and develop a care plan.
  • Coordinate with management on implementing and revising operational procedures related to quality client care.
  • Function as a resource and mentor to peers and subordinates to maintain professional nursing standards.
  • Act on client’s behalf to assure patient rights.
  • Be aware of home safety issues and reinforce good safety practices.
  • Respond promptly to client complaints concerning a caregiver, document complaints, and review with the Staff LPN and Director of Operations.
  • Lead caregiver trainings onsite at Senior Helpers of Northern Queens office, as needed.
  • Complete all paperwork in a timely manner.
  • Report any significant client changes to the office immediately.
  • Maintain current certifications, health requirements, and eligible working status.
  • Follow all policies and procedures as outlined in the Employee Handbook for Field Staff.
  • Perform other duties as assigned.
