The Sr. Population Health Specialist will act as a program/project coordinator for assigned initiatives, assisting leadership in the execution, evaluation, planning, and coordination of programs/projects' short/long-term goals to promote and achieve PHHEs goals within communities and healthcare systems. This role requires a strong understanding of how community-based influences, including social and structural determinants of health, impact child health outcomes. The specialist will identify and utilize appropriate project management tools and concepts, set priorities, track progress, and prepare reports for various stakeholders. They will also serve as the primary liaison for daily administration and operational issues, develop and implement policies, and support yearly operating budgets. Additionally, the role involves collaborating with quality improvement specialists and data analysts to develop innovative aims, measures, strategies, milestones, and goals for efficient data management, and supporting leadership in developing metrics around clinical equity, community health needs, challenges, and operational resources to reduce health disparities. The position emphasizes interdisciplinary collaboration, engagement with community members and organizations, relationship building, flexibility, adaptability to change, continuous learning, and a systems-learning view.
