About The Position

Requirements

Excellent written and verbal communication skills, with the ability to communicate clearly and effectively with a variety of stakeholders.
A commitment to understanding and responding to the needs of patients, families, referring providers, and internal teams with professionalism, sensitivity, and responsiveness.
The ability to assess situations, anticipate potential challenges, and independently identify effective solutions.
Strong skills in planning, prioritizing, and managing multiple responsibilities while adapting to changing priorities and maintaining quality and efficiency.
The ability to remain calm, attentive, and professional when managing complex or high-pressure situations.
The ability to work effectively with external referring providers, internal clinical teams, and other stakeholders to support a coordinated referral experience.
High school diploma or GED required.
4+ years or 2+ years with a bachelor's degree required.

Nice To Haves

Ability to communicate effectively in Spanish, both verbally and in writing, preferred.
Experience interacting with patients and/or physicians preferred.
Experience with relational databases, database structures, or data management methodologies preferred.
Bachelor's degree preferred.

Responsibilities

Support the daily operations of the Physician/Patient Referral Office with all referral communication tools: i.e. referral phone lines, pager, and electronic referral submissions.
Coordinate external communications related to physician and patient referrals and provide responsive, high-quality service to patients, families, referring physicians, and other stakeholders.
Maintain accurate patient and referral information within departmental databases and systems.
Facilitate the timely exchange and distribution of relevant clinical information between external referring providers and internal clinical teams.
Address complex patient and stakeholder questions and concerns, identify effective solutions, and escalate issues as appropriate.
Manage competing priorities, adapt to changing needs, and identify opportunities to improve processes and service delivery.
