About The Position

Employees in this classification are experienced LVN's responsible for performing a wide variety of patient care activities in an assigned specialty or subspecialty outpatient clinic. Work is performed under the direction of a staff nurse and/or physician as part of a total health care team in providing complete evaluations to patients. Works under the direct supervision of staff nurse or physician. No delegated supervisory authority; may instruct co-workers and supervise them in the performance of special procedures. Work is performed under usual clinic conditions, necessitating a high degree of contact with patients, families and clinic personnel.

Requirements

  • Completion of a Board of Nursing accredited program of vocational nursing.
  • Current Vocational Nursing license in the state of Texas or the attainment of a temporary permit from the Board of Nursing to practice vocational nursing pending full licensure in the state of Texas.
  • At least two years of LVN experience in a hospital, clinic or community-based environment.

Responsibilities

  • Monitor and assess patient generated health data received through the Remote Patient Monitoring platform, including vital signs and other condition specific measurements, in accordance with established clinical protocols.
  • Initiate appropriate patient outreach, intervention and escalation to the RN, provider or other member of the clinical team as indicated.
  • Conduct telephonic patient assessments and follow up, gathering relevant information regarding symptoms, medication adherence, vital sign readings and other factors that may affect the patient's condition.
  • Provide patient education and coaching regarding proper use of equipment and symptom awareness, lifestyle recommendations and when to seek medical attention, within the LVN scope of practice.
  • Document patient interactions, clinical observations, interventions, education, escalations and outcomes.
  • Coordinate care with clinics and providers to promote continuity of care.
  • Assist patients with barriers to successful participation in the RPM program.
  • Serve as a clinical resource and mentor for support staff.
  • Assist leadership with reviewing RPM workflows and identifying opportunities for process improvement.
  • Participate in quality improvement activities and measures.
  • Maintain patient confidentiality and adhere to HIPAA policies and requirements.
  • Demonstrate professional communication, critical thinking, accountability, teamwork and sound clinical judgment in interactions with patients, caregivers, providers and members of the health care team.
