Responsibilities include monitoring client medication, administering health screenings, and developing individualized rehabilitation plans (Service/Support Plans) in consultation with the treatment team. The role also entails delivering various community rehabilitation services and housing support services, documenting client progress, training clients in safety procedures, preparing residents for independent living, facilitating crisis intervention, and maintaining client charts. The position may also involve staffing the Emergency Phone, overseeing apartment maintenance, attending in-service training, providing coverage for other staff, serving as a 'Qualified Mental Health Staff Person', and performing other duties as assigned by the supervisor.
